Continuing from the previous issue, Thomas Wayne Batman arrives at Wayne Manor to find the aftermath of Jim Gordon's previous encounter with the Martha Wayne Joker (Flashpoint: Batman - Knight of Vengeance #2) in which she had tricked Gordon into shooting Harvey Dent's young daughter Debbie by disguising her as the Joker preparing to shoot Dent's son, Dexter. The Joker had then cut Gordon's throat with a razor while his guard was down looking after the children. While Batman is giving aid to the children, Dent's daughter Debbie remarks that she feels cold which triggers a flashback in Batman of his son Bruce saying the same thing while dying after being shot by Joe Chill, in the course of mugging the Wayne family 30 years prior. The flashback shows how Thomas shouted angrily at Martha to go find help while she was reeling in shock. Eventually she returns with a police officer but starts to wail "No!" when she realizes Bruce is dead. In the present day Joker leaps onto Batman's back and beats him repeatedly with a ball-peen hammer, accusing him: "how dare you". Another flashback shows Thomas pressing Martha about still not moving on from Bruce's death "months" later. When she replies that she misses Bruce, Thomas responds that he misses him too, "along with your smile". He then appears to tell Martha about his intent to seek vengeance on Joe Chill personally. A wordless sequence shows Thomas paying cash to a roughly dressed man for information on Chill's whereabouts. Thomas locates Chill leaving the Gotham Arms Hotel and follows him, pulling out a medical syringe. He then drops the syringe, calls out Chill's name and then beats Chill to death with his bare hands. Thomas returns to tell Martha of the act, only to find she has given herself a Glasgow smile and cracks the joke "I understand.... see? I'm smiling", revealing her self-inflicted disfigurement. She is then seen being dragged away by what appears to be medical orderlies, implying she has gone insane and must be institutionalized. Flashback ends, and Batman and Joker, still fighting, fall out of the upper story window onto the grounds below and continue to verbally and physically fight. Joker shows contempt for Batman's failures and violence, saying it changed him. Batman says the violence has changed nothing. They then segue into the topic of Batman having the opportunity to change time (mainline Flashpoint story) such that Bruce lives and the Waynes perish. He asks her if that's what she would want; she asks him to promise he'll do it; he says "I do", and Batman and Joker kiss. Joker then asks Batman to tell her what happens to Bruce in the timeline where he lives. When Joker realizes that Bruce becomes Batman instead of Thomas, she becomes hysterical and sprints away, falling into a hole that opens into the caves beneath Wayne Manor. Issue ends on a panel showing Joker splayed out at the bottom of the cave, either severely injured or dead.
